They stall because the leadership environment surrounding the work becomes misaligned.

Accountability becomes assumed rather than explicit.
Governance processes exist but do not drive real decisions.

Over time this creates friction across the system. Delivery slows, duplication increases and leadership teams find themselves working harder while progress becomes less visible.

What appears to be a performance problem is often a leadership alignment and structural clarity problem.

Without clarity at the top of the system, even highly capable organisations struggle to execute complex change.
